What Is PANDAS and Why Sudden Changes in Kids Should Never Be Ignored

If your child seemed to change overnight after being sick, this is something every parent should know about.

Today I want to talk about PANDAS. PANDAS stands for Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections. And while that name sounds big and intimidating, what parents really need to understand is this: PANDAS is a condition where, after a strep infection, the immune system can become misdirected and begin affecting the brain instead of simply fighting off the infection.

And when that happens, the changes can be sudden.

That is one of the biggest clues with PANDAS. Parents will often say, “My child was doing okay… and then all of a sudden everything changed.” Sometimes it shows up as OCD behaviors. Sometimes it is tics. Sometimes it looks like anxiety, emotional ups and downs, aggression, sleep struggles, sensory issues, regression, or even changes in handwriting and school performance.

And that is what makes PANDAS so confusing for families. Because it is not a slow build. It can feel abrupt. It can feel dramatic. And it can leave parents feeling like they are scrambling to figure out what just happened to their child.

Now, from a traditional standpoint, PANDAS is connected to strep. The idea is that the body creates antibodies to fight that infection, but in some kids those antibodies can mistakenly react with parts of the brain, especially areas involved with behavior, motor control, and emotions. That is why the symptoms are often neurological, behavioral, and emotional all at the same time.

So this is not just a child “acting out.”
It is not just a phase.
And it is not something parents are imagining.

There is a real reason these kids can look so dysregulated, overwhelmed, and unlike themselves.

But here is the part that many families are not told often enough.

Strep may be the trigger, but it may not be the whole story.

Not every child who gets strep develops PANDAS. And that matters. Because it tells us there is more going on beneath the surface. In many cases, the child may have already had underlying nervous system stress, dysregulation, or poor adaptability before the infection ever happened. Then when you add the infection and the immune response on top of that, it creates a much bigger storm.

That is why families feel like they stay stuck on a rollercoaster.

They are doing diet changes, trying supplements, and addressing inflammation. They are working so hard to help their child. And while those pieces can absolutely matter, sometimes the nervous system stress underneath it all is still there. If the body is stuck in a chronic state of stress and dysregulation, it becomes harder to regulate behavior, emotions, sleep, sensory processing, and healing overall.
